Dr. Kurt Black is providing custom fitted mouth guards to all CV Football Players. You must make an appointment in order to get one. Dr. Black's office has opened up September 3rd from 9am-noon for appointments.
Go to the football website at http://www.cvraiderfootball.com/ and click on the "Custom Mouthguard From Dr. Black" link and print out the consent form and fill it out. Then use the phone number on the consent form and call and make your appointment. HURRY, appointments will fill up fast.
